Consent terms & conditions for Client Conduct
-I agree that I am 18+, or am the parent/guardian, and voluntarily elect to receive a facial treatment.
Client Code of Behavior and Terms of Service:
1.Respectful Behavior: I agree to maintain a respectful and courteous demeanor during my visit to the spa. I will refrain from using offensive language, engaging in disruptive behavior, or displaying aggression towards employees or other clients.
2.Communication Standards: I will communicate with the spa's employees and fellow patrons in a polite and respectful manner. I understand that clear and appropriate communication contributes to a positive spa experience for everyone.
3.Handling Mistakes: In the event of any error or mistake made by the staff, I will remain patient and understanding. I will allow the staff the opportunity to correct any issues without resorting to yelling, offensive language, or any form of physical behavior.
4.Staff Safety and Comfort: I acknowledge that any behavior perceived as threatening or harmful towards the staff will not be tolerated. If a staff member feels threatened or uncomfortable due to my actions, they have the right to refuse service and request me to leave the spa premises.
5.Conflict Resolution: If I experience any dissatisfaction or feel unjust during my visit to the spa, I commit to addressing my concerns by sending a direct email to"kaividaspa@gmail.com." I will provide detailed and constructive feedback to assist in resolving any issues.
6.Refraining from Negative Posts: I agree not to post any negative reviews, complaints, or comments regarding the spa on the internet, including but not limited to social media platforms, websites, and applications. I understand that this helps maintain a positive image and reputation for the spa.
By continuing with my visit to Kai Vida Face Spa, I confirm that I have read and understood the Client Code of Behavior and Terms of Service. I agree to abide by these terms to ensure a harmonious and pleasant experience for myself and others at the spa.
- I understand that if I have a certain medical condition, such as abrasions/broken skin, bruising/swelling, dermatitis, psoriasis, recent sunburn, medications that affect treatment and any condition that may affect treatment needs to be disclosed and I understand that treatment may be declined to ensure my safety, Kai Vida Face Spa and other clients.
-I understand that results may vary per client and there are no certainties to result of treatment. Multiple sessions may be required to achieve maximum results.
-I agree that I have answered all questions regarding my health history, lifestyle and skin history to the best of my knowledge and abilities, and hold "Kai Vida Face Spa" harmless for any adverse reaction resulting from unknown medical conditions and or non-disclosed medical conditions that may be aggravated by the facial treatment.
